1- The Winning Customer Must Fill out the Back of the Winning Ticket.

Name, Phone #, Home Address, and Signature.

2- Scan the Ticket and Make Sure you Can Cash it. Than Press F1 to Accept Cashing it.

the Machine will Spit out an Authorization Number.

3- The Customer Must Completely Fill out and Sign the Lottery Claim Form.

 "Found in the Lottery File in the File Cabinet".

4- Write The Ticket Authorization Number you got from the Lottery Machine.

4- Call The Kentucky Lottery Corporation at: (888) 988-2543

                        Say that you are Retailer # 010817 and Have a Big Winning Ticket.

5- Give the Lottery Representative all The Information He needs To Process.

Follow His Instructions.

6- If the Customer Passes the Government Check, Approved, and the Kentucky Lottery Representative Prompt's you to PAY OUT the winner. he will have to Give you a CLAIM NUMBER Issued by the Lottery System.

7- Make Sure you Write the CLAIM NUMBER on the Winners Claim Form.

8- The Customer Does not Get any Copies We Keep The PINK Copy Drop it with your Paper Work, and Send the WHITE & YELLOW Copies to the Kentucky Lottery.

You Will Find The Kentucky Lottery Postage Paid Envelopes in the Lottery File In the File Cabinet. 

9- The Kentucky Lottery Postage Paid Envelope will have to Include:

               A-Claim Form Both White and Pink Copies.

               B- The Original Lottery Winning Ticket.

               C- Authorization Slip you Get from the Machine.
